Portability and Accountability Act (HIPAA) of 1996. HIPAA protects the privacy of individually
identifiable health information or protected health information (PHI).
It is the policy of Right Step Therapy, LLC that all providers and staff preserve the integrity and
the confidentiality of PHI pertaining to our patients. Employees of Right Step Therapy, LLC will
adhere to the standards set forth in the Notice of Privacy Practices and will collect, use, and
disclose PHI only in conformance with state and federal laws and current patient covenants
and/or authorizations, as appropriate.
Right Step Therapy, LLC takes your privacy seriously, and its providers will treat all protected

Right Step Therapy, LLC utilizes a HIPAA compliant electronic medical record system to collect,
use, disclose, and store PHI. During the registration process, we must enter personal
information such as name, address, phone number, social security number, and insurance
coverage information.
Right Step Therapy, LLC may use or disclose my personal health information for the purpose of
carrying out treatment, collaborating with other health care practitioners involved in my care,
obtaining payment, and any administrative operations related to treatment or payment.
Patients have the right to request restrictions, in writing, regarding how personal health
information is used and disclosed for treatment, payment, and administrative operations.
Right Step Therapy, LLC will never rent or sell PHI or share your PHI for marketing purposes.
PHI may be shared with family members, guardians, or specified individuals when requested
only with verbal and/or written consent.
Patients of Right Step Therapy, LLC may request a copy of your medical records by contacting us

request or as required by the law.
